The purpose of the questionnaire is to determine whether there may be any relevant health problems that could affect an employee’s ability to undertake the duties of the post they have just accepted, or place them/others at any risk in the workplace. Should such health problems be identified, one of our case managers will review and provide reasonable adjustment recommendations where appropriate.
